Transitions Care Officially Licensed in Northwest Ohio
(Toledo, Ohio – October 25, 2022) — Transitions Care has been approved to begin caring for patients in the state of Ohio, based out of our office in Toledo, Ohio. Through this expansion, we will be serving patients in Lucas, Ottawa, Fulton, Sandusky, Wood, and Henry counties. The addition further expands Transitions’ Midwestern presence to provide both hospice and palliative care services to a growing number of patients in the state of Ohio. With their unique 24/7 care model, Transitions aims to be a leading provider in the area upon arrival.
