Make an Impact at Transitions
At Transitions, our teams emphasize our values: To empower our patients and families by providing the tools and support necessary to embrace life. We don’t focus on the finality of death; we focus on the practical side of living. Our job is to assist our patients and families through the physical, emotional, and spiritual challenges of life’s final challenges. We understand that to our patients and their families, it’s about living! If you are caring, compassionate, and want to make a positive difference in someone’s life, Transitions is the place for you.
Testimonies from our Team
“I love the autonomy of Transitions, I am able to schedule my day and do things that work for me in order to get out there and do things most effectively. Transitions is willing to let you be a human being and have good days and bad days. They care about you as a person and if you say something - they listen. Transitions work around it and try to ease the burden. A professional working relationship, but with a heart. Transitions tries to get people to be their best selves and they allow them to make mistakes and are willing to work with you to move forward.”

Why work at Transitions?
Transitions lives by three commandments that our employees embody each and every day – treat each patient uniquely, commit to care, and always do the right thing. These tenets are present in our interactions with not only patients and their families, but between coworkers and team members. We treat each other with respect and kindness, and are always open to new ideas, backgrounds, and perspectives within our teams.
Because of these commitments, Transitions operates as a culture dedicated to care. This is felt across our organization, from our executive leadership to our teams in the field. We are truly making a difference in the lives of our patients and their families, making the work all the more rewarding for our teams. End-of-life care takes a special type of person, and at Transitions we embrace the natural gravitation towards caregiving that makes our teams so impactful at what they do.
Our Three Commandments
All of our team members pledge to fulfill the Transitions Commandments. These three fundamental principles ensure that each patient and family receives the focused and individualized care they deserve.
Each patient is unique
Transitions believes care plans should never be “one size fits all.” Understanding that all of our patients have their own personal needs enables us to develop a customized plan of care that respects their specific wishes.
Commit to care
By committing to care, each team member pledges to dedicate themselves to their patients and the uncompromising demands of end of life care, symptom management, and primary care. Our staff is always available. We will tend to our patients as often as needed regardless of their location — day or night.
Always do the right thing
Doing the right thing and doing the easiest thing are rarely the same. When it comes to Transitions Care, team members are committed to continually dedicate the time and energy necessary to always do the right thing.
Transitions offers many opportunities for growth. Many of our Registered Nurses and Regional Directors of Nursing were formerly CNAs with the company who furthered their education and grew from within the organization. Transitions offers tuition reimbursement for those who want to further their education while working for us because we believe that investing in our team members will lead to success for Transitions and greater clinical outcomes for our patient population. If you would like to learn more about this program and apply for a position at Transitions, please be sure to ask your interviewer about tuition reimbursement for your position.
Equal Opportunity Employer
Transitions is an equal opportunity employer that is committed to inclusion and diversity. We take affirmative action to ensure equal opportunity for all applicants without regard to race, color, ancestry, religion, sex, national origin, sexual orientation, gender identity, age, marital status, disability, veteran status, or other legally protected characteristics.
Transitions will not discriminate or retaliate against applicants who inquire about, disclose, or discuss their compensation or that of other applicants. We also consider qualified applicants regardless of criminal histories, consistent with legal requirements.
Transitions is committed to working with and providing reasonable accommodation to applicants with physical and mental disabilities.
Benefits and More
We also offer competitive salary and benefits in a team-oriented work environment. Our healthcare insurance is through United Healthcare, and various plans and options are offered to all of our employees depending on their needs. We make sure that all of our benefits, pay, time-off and insurance policies are consistently updated to fit the unique needs of our employees.
Testimonies from our Team
"When our patients are vulnerable or fatigued, we get to be one on one with them in order to help them navigate the situation. You don't get that anywhere else as a nurse. You get to know who the patient is and the opportunity for human connection that you get as a nurse at Transitions is unlike any other. Everything we do is for the patient and their family. The best thing that you can do is to know yourself and to have an open mind when you come into hospice. This job is one of the most rewarding positions a nurse could have. The hospice world gives the most fulfillment out of any nursing career you could choose.”
