Transitions Medical Equipment provides customized equipment solutions to nursing homes, skilled nursing facilities, rehab facilities, and surrounding hospice communities across Illinois, Indiana, Michigan, and Pennsylvania. Our team members are professionally trained, knowledgeable of durable medical equipment, timely, polite, and hold years of industry experience. TME delivers directly to patient locations with 24/7, 365 service available to all patients. With over 3,000 patients served in 2020 and an even larger number served in 2021, TME is excited to continue providing high quality equipment to patients, ensuring that they are able to remain in the comfort of their own home with a high quality of life even with a chronic or terminal diagnosis.

Prime Care Physicians is a medical practice that provides house-call services to homebound patients. Prime Care has been a leading provider in house-call services since 2012. Their services include in-home checkups, sick visits, podiatry services, wound care and debridement, transitional care management, and palliative care.
A majority of Prime Care’s patient base is in Chicago and surrounding areas. Along with Transitions, Prime Care services the following counties: Cook, DuPage, Lake, McHenry, Boone, Kane, Kendall, Grundy, Kankakee and Will. Both Prime Care and Transitions are dedicated to providing quality care through communicating with all parties involved in a patients’ care, and are always able to schedule referrals within 24-48 hours.

TUMI Health is a primary care clinic with emphasis on the prevention of chronic illness through adequate nutrition and weight loss.
RevUp is a software used by Transitions that offers palliative patients a personal Health Coach that will help them successfully achieve their goals by managing and reporting on aspects of day-to-day health. This information is constantly monitored by Transitions’ Symptom Management Team, providing our patients with live, up-to-date information on their health and current outlook. This allows us to visit with patients in a timely and efficient manner when they need us most, as well as help Transitions monitor our patients even when we are not present – furthering our promise of 24/7, 365 palliative and primary care.
