Q: Can you please provide a little more information about the data analytics platform Transitions is using?
A: Trella Health is a multipurpose platform in which metrics are provided that help to accelerate organization growth. The information is presented in a user-friendly way that makes understanding and digesting data easy. Trella utilizes 100% of medicare fee-for-services to bring its clients the most up-to-date analytics. Trella Health works to provide individuals using their platform the best insights and visual representations of information gathered by Medicare A + B. The data this platform provides falls into five different fields; home health, hospice, skilled nursing, accountable care organizations, and health systems. Data can be drawn from any of these five solutions, giving users a comprehensive picture of the current and potential markets for each of these categories. The platform in which Trella compiles and represents their data is Marketspace.
Q: How does Marketspace support Transitions and those we serve?
A: Marketspace is a platform in which viewing data is digestible. When viewing the main page of a Marketspace account, there are three different options in which analytics have been compiled; physicians, facilities, or hospices. At Transitions, we utilize this information to provide in-depth information to support our internal team and our referral partners. A big benefit of using Marketspace is its infographics and data dashboards. The infographics are very useful to provide aggregated information and key insights into how Transitions’ processes support those we serve with care. It also provides some information related to physician referrals, and the percentage in which a specific hospital refers their patients to a company, allowing for data-driven decisions and help with building stronger referral relationships. Another benefit is that care information is tracked to afford greater communication between Transitions and the referral source while a patient is using hospice services.
I have the confidence that this platform will help drive growth both in establishing new territories as well as solidifying our current relationships with various healthcare providers.
