As 2022 quickly approaches, Transitions is expanding its primary care services to supplement our symptom management efforts and reach a growing number of home-bound patients in all areas we serve.
As 2022 quickly approaches, Transitions is expanding its primary care services to supplement our symptom management efforts and reach a growing number of home-bound patients in all areas we serve. Our physicians and APN’s are readily available to conduct primary care visits directly in the comfort of our patients’ homes. We continue to embrace our mission to provide 24/7 spiritual, emotional, social, and physical support for our patients – no matter the type of diagnosis or progression of symptoms.
Our primary care services include management and treatment of acute and chronic diseases, medication management, health promotion, disease prevention, and immunizations, all in the comfort of your home. Patients may receive primary care services in conjunction with Transitions’ palliative care and symptom management.
Unlike most primary care providers, Transitions Care is equipped to provide advanced symptom management with the capabilities necessary to optimize care coordination while maintaining strong and unique relationships with each patient. At Transitions, our primary care services bring the doctor to you. Our primary care involves day-to-day 24/7 healthcare where the provider acts as the patient’s first point of contact in the health system.
Through our primary care services, our dedicated teams of doctors, nurse practitioners, and physician assistants provide care for individuals who have difficulty going out or traveling to see the doctor. Our goal is to allow home-bound patients to continue receiving regular physician services and to prevent any future hospitalizations through our symptom management capabilities.
Currently serving patients in Central Illinois, Chicagoland, and Indiana, we will also begin serving primary/palliative patients in the Pittsburgh area in January 2022 and in Michigan in February 2022. In addition to our current oncology model services, we will be expanding to three other large oncology groups in the Chicagoland area over the next few months.
In January of 2022, we will be moving over to a new EHR (Electronic Health Records) platform called NextGen. NextGen is an award-winning, cloud-based EHR specially designed to provide the highest quality of care, improving the experience for both patients and providers on one integrated platform. It includes a convenient patient portal and revenue cycle management system to provide a clinical and billing solution with mobile accessibility. Charting is made easier by type, talk, or touch. Reporting, analytics, and communication are also improved for a more efficient work flow.
We also recently implemented the Acclivity Health platform, which utilizes analytics, machine learning, and workflow management to help avoid unnecessary ER visits and hospitalizations. This platform will help Transitions improve quality by delivering value-based care, providing appropriate and timely services, and reducing health care spending for the sickest patient population.
