Transitions Expands Services into Southwestern Pennsylvania
Transitions is happy to announce that it will begin serving the Pittsburgh, PA area starting October 2021.
(PITTSBURGH, PA – October 4th, 2021) — Transitions has acquired Pilgrimage Hospice, founded by former NFL quarterback of the Pittsburgh Steelers, Bruce Gradkowski. Starting October 4th, 2021, based out of the former Pilgrimage office in Pittsburgh, Transitions will begin providing both hospice and palliative care services to patients in Pennsylvania. Both companies hold a similar philosophy in care – compassionately providing a personalized and patient-focused experience. Through this expansion, Transitions will be serving patients in Allegheny, Beaver, Butler, Fayette, Washington, Lawrence, Armstrong, and Westmoreland counties in Pennsylvania. The addition extends Transitions’ presence beyond Illinois, Indiana, and Michigan to reach a growing number of patients in the Midwest region.
