Each patient is unique
Transitions believes care plans should never be “one size fits all.” Understanding that all of our patients have their own personal needs enables us to develop a customized plan of care that respects their specific wishes.
Commit to care
By committing to care, each team member pledges to dedicate themselves to their patients and the uncompromising demands of end of life care, symptom management, and primary care. Our staff is always available. We will tend to our patients as often as needed regardless of their location — day or night.
Always do the right thing
Doing the right thing and doing the easiest thing are rarely the same. When it comes to Transitions Care, team members are committed to continually dedicate the time and energy necessary to always do the right thing.
Growth
Transitions offers many opportunities for growth. Many of our Registered Nurses and Regional Directors of Nursing were formerly CNAs with the company who furthered their education and grew from within the organization. Transitions offers tuition reimbursement for those who want to further their education while working for us because we believe that investing in our team members will lead to success for Transitions and greater clinical outcomes for our patient population.
