All of our team members pledge to fulfill the Transitions Commandments. These three fundamental principles ensure that each patient and family receives the focused and individualized care they deserve.
Commit to care
By committing to care, each team member pledges to dedicate themselves to their patients and the uncompromising demands of end of life care, symptom management, and primary care. Our staff is always available. We will tend to our patients as often as needed regardless of their location — day or night.
Each patient is unique
Transitions believes care plans should never be “one size fits all.” Understanding that all of our patients have their own personal needs enables us to develop a customized plan of care that respects their specific wishes.
Always do the right thing
Doing the right thing and doing the easiest thing are rarely the same. When it comes to Transitions Care, team members are committed to continually dedicate the time and energy necessary to always do the right thing.
We love hearing from you!
"The high professionalism of each staff member was impressive. Every single one who entered into the apartment brought with them warmth, joy, and a smile. The attitude was always one of wanting to help my mother. She was seen as an individual and not as an "old woman" whose life was no longer of value. I want you to know I am thankful for all of you. I treasure each one of you and I hope your holiday will be filled with joy."
