Transitions offers the highest quality of end of life care and palliative care services. We specialize in supporting individuals and families who are facing the physical, emotional, social, and spiritual challenges confronted when dealing with terminal and chronic illnesses. Paramount to our philosophy is to ensure that our patient is experiencing their final transition on their own specified terms — in comfort and with dignity. In our care, our main objective is always to maximize patients’ comfort and quality of life.
For us, whether it is end of life or palliative care – it is always about living. That is why we provide patients with a comprehensive care plan led by a dedicated interdisciplinary team.

Leads the way to ensure each patient is receiving the proper medications and medical treatments during their course of care with Transitions.
Our nurse practitioners work with our physicians to develop a comprehensive and individualized care plan for each patient.
Our nurses, who are available 24 hours a day 7 days a week, work with patients to execute a plan of care that will make them the most comfortable in hospice.
CNAs are the ‘guardian angels” that assist with the patients personal care, as well as emphasize family support.
Our dietitians ensure patients are receiving the nutritional intake that best fits their plan of care.
Our chaplains provide the spiritual and emotional support that many patients seek at the end of their lives.
Volunteers enrich our patients’ lives by bringing them the things that matter most to them directly to their bedside.
At Transitions, we offer a variety of services beyond our clinical care. These include our volunteer services, chaplaincy support, diet and nutritional advice, Music Therapy, Massage Therapy, Pet Therapy, and more!
Transitions’ bereavement team specializes in a variety of grief interventions to support our patients and families throughout the course of care, providing services regardless of the stage or severity of grief someone may be experiencing.

End of Life Care
Transitions offers all levels of End of Life Care – routine care, continuous care, inpatient care, and respite care. End of life care services are covered by the Medicare Part A Benefit, Medicaid, and most private insurance plans, often coming at no cost to the patient. Our end of life care teams are dedicated to providing high-quality care, with the goal of maximizing patients’ comfort and quality of life during the course of care.
Palliative Care
Palliative care takes a team approach to treating patients with chronic and/or terminal illness in a home based setting. It optimizes quality of life by anticipating, preventing, and treating suffering. Palliative services are appropriate for any person that has a chronic and/or terminal illness, regardless of life expectancy, in which additional support is needed. Palliative care services can be provided in any setting and for a variety of different diseases and symptoms. At Transitions, our main focus is preventing repeated hospitalizations, more effectively treating long term illness, managing suffering, and focusing on the quality of life for all patients.
