Transitions Care Acquires Michigan Based Cardinal Hospice

Transitions Care is excited to announce its acquisition of Cardinal Hospice, effective November 11, 2022. Through this acquisition, Transitions expands its Michigan service area from Detroit and Kalamazoo to include Cardinal’s current locations in Bay City and Oxford. The expansion bolsters Transitions presence across Michigan and allows for extensive end of life care coverage throughout the state. The purchase of Cardinal Hospice allows Transitions to expand its footprint throughout various markets in Michigan and provide a platform to add Palliative and Primary care services in conjunction with Hospice services. “We are excited to welcome the Cardinal Hospice employees to Transitions Care”, says Transitions Care Chief Operating Officer, Rick Cantrell. “Their passion for and dedication to providing high-quality care to patients and their loves ones is a perfect fit for our team as we continue to expand our hospice service line” Transitions offers the highest level of hospice and palliative care in each of the areas we serve.
