Transitions Care Expands into Toledo Metropolitan Area

Starting in April 2022, Transitions Care will begin providing primary care, symptom and chronic care management, and end of life care services to patients in the Toledo, Ohio metropolitan area.
Transitions Care offers the highest level of primary care, symptom and chronic care management, and end of life care to patients in the comfort of their own home. Since its inception in 2007, Transitions has continued to grow with a patient-centered mindset. Each patient and their family receives a customized care plan and a dedicated team of caregivers, which includes nurses, nurses aides, chaplains, nurse practitioners, dietitians, bereavement specialists and social workers – all of whom are available 24 hours a day, 7 days a week, 365 days a year.
